In order to ensure the safety of our patients, visitors, and staff we have added a mobile screening office. This mobile unit will be easily identifiable and is located in the back of the facility. We are asking ALL patients to call 815-772-5511 between the hours of 8:00-5:00 Monday through Sunday before you leave your home. A healthcare provider will screen patients over the phone and make recommendations based upon the phone screening.
We want to thank our patients and community members for assisting us in keeping our communities healthy. Both our primary and specialty clinics will continue to use telehealth options whenever appropriate to maintain the social distancing requirements.
