If you are exhibiting symptoms of fever, cough, sore throat, shortness of breath, diarrhea, vomiting, loss of smell or taste, and/or headache please call 815-772-5511    A receptionist will schedule an appointment and you will then be transferred to a medical provider for screening.


Morrison Family Care Clinic will have Pfizer Covid vaccine appointments available. Please call (815) 772-5537 to schedule. Come through the main entrance to the Family Care Walk-In Clinic. This is for ages 16 and over. If you require a special time before or after work hours we will try out best to accommodate your request.   As a reminder, the scheduling of vaccine administration is dependent upon vaccine availability.

Mobile Screening office

In order to ensure the safety of our patients, visitors, and staff we have added a mobile screening office.  This mobile unit will be easily identifiable and is located in the back of the facility.  We are asking ALL patients to call 815-772-5511 between the hours of 8:00 – 5:00 Monday through Sunday before you leave your home. A healthcare provider will screen patients over the phone and make recommendations based upon the phone screening. The Covid trailer is on the NW side of the hospital. Enter in the main entrance and follow the signs.

We want to thank our patients and community members for assisting us in keeping our communities healthy.  Both our primary and specialty clinics will continue to use telehealth options whenever appropriate to maintain the social distancing requirements.
